Camp LiveGirl Acknowledgement and Permission Slip

This letter is intended to both inform you and receive your acknowledgement and permission for your child to participate in Camp LiveGirl on July 20th through July 24th, 2020. Camp LiveGirl will take place at New Canaan High School (NCHS) in New Canaan, CT. Your child will be transported to and from LiveGirl Camp LiveGirl by:

Camp LiveGirl participants will be supervised from drop off (after 8:30 am) through dismissal time (4:00 pm) in the following ways:

LiveGirl Staff
By LiveGirl Camp Counselors and CITs; and/or
Volunteer parent chaperones.

All participants are required to abide by the established policies, rules and/or regulations set forth in this letter. If your child fails to abide by these policies rules and regulations, your child could be prohibited from continued participation and may be sent home at the expense of parent or guardian.

Your child’s participation in Camp LiveGirl on July 20th through July 24th, 2020 is voluntary. You authorize LiveGirl, Inc. to take and publish photographs and videos of your child for use in printed publications, website, and social media.

Your written consent on the application form is necessary for your child to participate.

You agree to the terms of this letter on your behalf and on your child’s behalf. You hereby release, waive, forever discharge, hold harmless and covenant not to sue LiveGirl, Inc., their respective directors, officers, trustees, employees, volunteers, and agents (collectively, the “Released Parties”) and their successors and assigns from any and all liability, claims and demands which you or your heirs, assigns, next of kin or legal representatives may have or which may hereinafter accrue with respect to any bodily injury, personal injury, illness, death or property damage which arise or may hereafter arise from or is in any way related to your child’s involvement in Camp LiveGirl, whether caused wholly or in part by the simple negligence, fault or other misconduct, other than intentional or grossly negligent conduct, of any of the Released Parties.

• You further agrees that if, despite this letter, you, or anyone on your behalf, makes a claim against any of the Released Parties, you will indemnify, save and hold harmless each of the Released Parties from any litigation expenses, attorneys’ fees, loss, liability, damage or cost which they may incur as a result of such claim.

• You recognize and fully understand that: 1) involvement with Camp LiveGirl may involve risks and dangers of serious bodily injury, including permanent disability, paralysis and death (“Risks”); 2) these Risks and dangers may be caused by your child’s own action or inactions, the action or inactions of other children’s involvement with Camp LiveGirl, forces of nature, the condition of the grounds, or the negligence of the Released Parties; and 3) there may be other risks and social and economic losses, costs, and damages your child incurs as a result of his or her involvement with Camp LiveGirl and you fully accept and assume all such risks and all responsibility for losses, costs, physical harm and damages your child incurs as a result of his or her involvement with Camp LiveGirl. You hereby expressly and specifically assume the risk of injury or harm in your involvement with Camp LiveGirl and release the Released Parties from all liability for any loss, cost, expense, injury, illness, death or property damage resulting directly or indirectly from your child’s involvement with Camp LiveGirl.

• You expressly agree that this letter is intended to be interpreted as broadly and inclusively as permitted by the laws of the State of Connecticut and that this Agreement shall be governed by and interpreted in accordance with the laws of the State of Connecticut. You further agree that in the event any clause or provision of this letter shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining clauses or provisions of this letter, which shall continue to be enforceable. A waiver of a right under this letter does not prevent the exercise of any other right.

BY PARTICIPATING IN CAMP LIVEGIRL, YOU (1) FULLY UNDERSTAND AND AGREE TO THE TERMS OF THIS LETTER, (2) UNDERSTAND THAT YOU HAVE GIVEN UP SUBSTANTIAL RIGHTS BY PARTICIPATING IN CAMP LIVEGIRL, (3) HAVE PARTICIPATED IN CAMP LIVEGIRL FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE OR GUARANTEE BEING MADE TO YOU AND (4) INTEND BY PARTICIPATING IN CAMP LIVEGIRL TO DEMONSTRATE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
